Part I: Complete registration online via SchoolMint
- Create a SchoolMint account at https://pusdschools.schoolmint.net/signup
- Fill out a registration packet which includes the following forms:
- (1) student registration
- (2) emergency/medical information
- (3) receipt of annual notification/parent signature page
- (4) school funding data collection form.
- After you submit your registration packet, you will receive a notification that online registration has been complete.
Part II: Visit school site to complete the registration process
- Bring all required registration documents: immunization record, proof of birth, parent/guardian identification, and proof of residency. Individuals other than parent/guardian must bring an appointed guardian/s court order to school office.
